What is a Health Risk Assessment (HRA)?
An HRA is an online questionnaire about to your health to determine if you should consider healthcare services for common healthcare needs and conditions, such as heart health, breast cancer, and other health conditions.
How much does it cost?
The HRAs are free. You will never have to pay for your results or to take it.
How long will it take to fill out?
It should take less than 10 mins to fill out & get your results.

We will send you an email with a link to finish it when you can.
What will you do with my information?
Your results are completely confidential. We will never sell or share your personal health information. If you request it, we will email your results to you. We may send you information specific to the HRA you took, such as tips to stay healthy, what a screening means, you can easily opt out of these emails at any time.
What should I do if I’m high risk?
After you receive your report you should talk to your primary care physician (PCP) about the results. If you do not have a PCP we can help connect you with one online.
What information is needed to take the HRA?
The HRA asks for basic information about a person such as age, height, weight, family history, tobacco use or alcohol use history, activity/exercise details, sleep patterns etc. If you do not have all the medical information requested you can skip or provide at a later time. Your information is confidential and will not be sold or shared with other organizations.
What happens after I take an HRA?
After completing the questionnaire, you will be provided with a personalized report about your risk for developing the health condition and/or tips for how your health can be improved. We will follow up with you via emails to help connect you with a physican or schedule a screenings if recommended. You can opt out of the emails at any time.
